I think you will find the Paladin can cast divine spells from the 4th level according to the D+D core manual,I seem to remember that was the case on other D+D games that I've played aswell. Hope that helps.

I think it happens if you kill someone you shoulden't as I killed a nurse in the Core district and everyone apart from the main NPC's had a free for all with me. I take it you mean to free Shaldrissa from her cell,if so 1) try opening the chest near the cell, or 2) you should be able to kill Yesgar who drops the cell key

Try using any henchmen/summoned monsters/familiars as meat machines while you pluck away with ranged weapons,if you can,get hold of some enhanced arrows or bolts.If you get hit too many times use the stone of recall and get lady A to heal you and go back to finish the job in hand.
