Figured it out....

I had to kill the Lich, then the other gate opened.

When I follow the knight in the Shadow Ruins, he seems to just walk and then stop. Am I supposed to run through the gates beside him when he stops?

If I do this, I end up in the islands and i cannot for the life of me find a away out!

Is there a pattern to these final islands?

Aye. I see the platform with the 2nd gate, but it is never open. Only to gate I came in from is open, even after I already activated the glyph.

Any other ideas on how to get the 2nd gate to open?

OK...In the Shadow Ruins (of course...) I have just finished crawling to the top of the stone pillar, while the wraith and the death knights have been throwing ice comets and blasts at me non-stop, and I activated the red rune button on the top. After this, there is only 1 gate open (on the bottom ...
