When I bought all my other spells they appeared there automatically. Is there a way to physically put a spell there?

Some wandering traders sell tatoo cards, and there is a tatoo artist in Knothole Glade, next to the armorer just inside the front gate.

You get the scars from fighting. Not from any specific attacks, just how many times you engage in combat. You get more scars the more you fight. It's inevitable. Unless you don't fight melee, you can't help but get scars.
