Are you sue you have done everything in the Mines of Kaderak? Sometimes the game doesn't register that you have completed a quest until you walk back to where the golden star is flashing. If this doesn't work, then go talk to Lord Kalrathia...even if he keeps giving directions to the mines, go talk ...

You need to go back into the garden of the ancients and search for the other Frozen Crypt which you haven't found yet. There will be one, you won't have explored everywhere! I remember that one of the crypts is further out of the way than the others, so search off the main path.
