Theoretically, with out ever losing any levels on your majors it is 38.5.
If you do lose levels and then train them back up the maximum level is 250.

Not only the nudity but sexual content is still there, some female NPCs will flirt with you, you can't follow up of course, also there is a inn where only sailors can sleep and if you go into a room the innkeeper is in bed with them(just laying beside but still).

You can also do it with scrolls, have a stack of scrolls, double click it and then drop the item you wanna dupe. Note:item number must be less then scroll number, (in inventory anyway, you can just drop them).

Difficulty only affects damage input/output, nothing else. Maybe you just had a bad run.

I have never been there before but if it says opened from somewhere else then maybe there is a switch or lever somewhere inside that room. I would just scan over everything to see before loading an earlier save.
