You've probably already figured this out by now. Spoiler ahead: Its Soegos (the dustman in Dead Nations) head found in his spoils after you kill him.

So I'm in the Maze of Reflections I finish talking to the incarnations then go to speak to Deionarra. After selecting to touch her hand to transport me to the roof it just places me back in the Maze of Reflections with Deionarra standing there repeating the previous conversation.
