Before you place the last tear summon what ever you can and place them 'beside the walls'. Cast Protection from Evil on your whole group and place the last tear. Be ready to pull back your fighters and heal them from time to time. Always keep someone on him.

Pull the mages back out of the fog of war, the ignore Irenicus and take all of the demons out first, I don't think that Irenicus really actively comes after you until the demons are dead. after you kill the demons, the rest should be a little easier.

I believe that Irenicus uses MISLEAD twice in the battle. The first and second slayers which show up don't do anything at all - they are decoys. Irenicus himself is invisible and protected inside the "floating sphere."
Don't waste resources on the first slayer - First concentrate on killing the demons. When the demons are dead, look around for the "floating sphere" which should be else where in the room - that is the real Irenicus. He will than cast MISLEAD again, producing a second decoy. Don't waste resources on it either - concentrate on the other stuff he conjures (a djinn, for example). Finally, a third slayer will show up. That is the real guy - hit him with all you have. He does have regeneration ability and lots of protections, so hit him with lots of protection and resistence removing spells.
I finally beat him by down by surrounding him with a Fire Elemental, two shambling mounds, a skelleton warrior, a nymph (casts mass heal spells), Minsc, and my main character. Irenicus was taking tremendous amounts of damage, and it still took forever for him to go down. I swear he was at "almost dead" for about 10 minutes.
