I've no idea how many times I've played this game, but certainly enough that I recently installed a couple of mods as I could sleepwalk through SoA. But one of the things that always makes me pause before skipping Irenicus' dungeon is that twice before I've found a surprise - once +2 chainmail, and once a +3 spear (found on a goblin just after the power cell room) that Jaheira is still happily wielding. The thing I remember about both is that they "fit" my protagonist at the time - the chainmail was when I had a bard, and the spear when I had a beserker with points in spear. So I wondered if that was a key in some "die roll" for what you found...

I've always had the latest patch, and only recently started using mods.

Yes, there's a mod which places new items in the game, I don't know it's name because I installed it long time ago, but I think it's one of the mods available from the the Pocket Plane Mod site.

I found a Two Handed Sword +2 on the bodies of the very first set of goblins I came across, west from that first Genie. I have TOB, and the only mod I have is the official patch. No Baldurdash or anything else. I've installed and uninstalled the game countless times, and I'm not sure whether this is the first time I've used a patch. It's the first time I've found anything like this. My protagonist is a Kensai, with *** 2-weapon style, and *** in Katanas, so it doesn't fit him. Minsc is currently using the sword.

For a character who has played through BG1 first. Certain of the items that are carried at the end of BG1 are carried through to the dungeon.
I once played a multi-player game with four characters that came from different BG1 games. They all had Balduran's helmet in BG1 with the result that I found 4 Balduran's helmets in the chest where there is usually one.

The only two I got from BG1 (aside from the helm) are the claw and the pantaloons.

I did a search, and the only reference to a +2 longsword is in "IMPORT02.2DA". Is it safe to say you imported the character that found the sword? If so, this is your answer. A Long Sword +2 cannot be created randomly (Latest official patch, no mods) as far as I can tell.

I used a brand new character, no mods, and no patches when I found the sword.
