The first time I created my Swashbuckler, and once I finally got a short bow, he would run up to point-blank range before firing it. This one took a while to figure out. I guess I never had him go off by himself, but I began to wonder....
Finally, I sent him ahead to scout an unexplored area, and, sure enough, he cut a shoulder-wide tunnel through the darkness. He was near-sighted! Seems like a zero was dropped somewhere in the code. Instead of seeing, say, 20 feet, he saw 2.
Before you ask, party infravision was ON. And even in the midst of the entire party, with everything clearly illuminated, he still ran right up to his opponent with his bow.
Weird, huh?

Actually, I had this same problem with a monk with a sling. I know he was not blinded as I was soloing and I could see the enemy. I just quit the game for a little while and when I started again it was fine. I don't remember what enemy I was attacking when this happened. Do you remember what yours was Saigo? Or where? Just looking for a connection.
