You get it from the Ilithid mini-dungeon in the sewers. You will need the Sewer key to let you in.

Well, The Hammer Of Thunderbolts is in some mind flayer dungeon beneath the temple district.
It's a hidden door, and you will need a key to open it. The key can be found on Tazok in Windspear Hills.
The illithids shouldn't be a pretty tough fight, just send your mage in front. Good Luck.
