I'm 4th level now, but my fighters NEVER hit on more than 10% of their attacks. Not even if I have proficiency, bull's strength, etc.
Trip attack NEVER works.
some monsters attack with virtually 100% accuracy (think Hill Giant)
I've had instances where my Paladin will miss on 30-35 straight attacks in mutliple encounters. This is almost mathematically impossible.
is this game seriously broken?

Check your roll window break-downs to see what's going on.
If you don't know how, open the roll window and click the blue text that says "misses" or "hits" to see the numbers and how they worked out.
The random number generator isn't the best (which is?), but it's not THAT bad. I suspect there's some penalty or something you're failing to account for. (Maybe you're using a bastard sword one-handed without the exotic profiency, for instance.) The roll window will show you what you need to get, what you're rolling, and what the modifiers are. I've often rolled several 1's in a row, and I roll more 1's than 20's by far in ToEE, but I don't miss 20 times in a row.
I have had 16 consecutive d20 rolls below 10, however. That's pretty unlikely, mathematically, so the game's not perfect in this regard, but still.
As for the hill giant, he's got like +18 to hit or something, so unless you've got a 20 AC or better, he'll only miss on a roll of 1. He's tough for a low-level party.

I can't get by the broken tower because I simply can't hit anyone. If I miss once, the game goes into some kind of statistical reduction, where my chance of hitting is reduced every round afterwards. I have 7 people in my party, including 4 fighters. I checked the proficiencies -not problem.
In my last battle I missed 26 rounds in a row with every character. I was totally destroyed by the men in the tower.

It'd be nice to see just what the heck the problem is. If you're rolling 1's every time, that's a problem. If you're rolling enough to hit, but aren't hitting, that's a problem. If the enemy's AC is absurdly high, that's a problem.

It'd be good to know which, and maybe we can set about fixing it. Whatever this bug is, if it is one, there must be a cause and a way to fix it, because this doesn't happen to other people.

By the way, what patches are you running? If the answer is "none" well we know the first thing you need to do right off.

As I was well into the game with my first party before I learned how badly the patches were needed. I decided to try and finish my first game unpatched (I know I am asking for trouble LP but have dodged the bullet so far) and have been able to work around most bugs. I too noticed some disparity in rolls from time to time. What "usually" worked for me was to reload the game (that seems to reset the random generator) if it appears that it is rolling consistantly low. I save frequntly (and especially do so between each character when I level them up).
