I've been a while without playing bloodlines, last time I had the unoficial patch v1.7. Now I got the v2.5, but the occult itens have beem moved from its original places. I've found the Heart of Eliza and the Galdjun, but I couldn't find the second item that used to be sold in Tseng's (I don't remember the name). Does anybody know where it is? Do you have a list of where all of the occult items have been placed?

This may be a bug. Taken from the 2.1 patch notes:
Mr. Ox does give you the item formerly found in the warrens, and one of the items that used to be sold by Tseng (the +1 melee item, specifially). But the +1 dex item from Tseng seems to be missing. At least, I never acquired it in my most recent game.Moved occult items from warrens and Tseng's to be rewards of Mr. Ox.

You are probably referring to the Fae Charm: Isaac will give it to you as a reward for driving the Tzimisce away after you have survived the Mandarin's tests in chinatown.
