I named my half-orc fighter Valentine, which I thought sounded realy cool and strong. I had a dwarven barbarian named Gavrelian.
That is just my own two cents.

For dwarfs i use secondnames such as:
Thunderclap
Orcslayer
Fireaxe
Longbeard
and some more...

I have a number of approaches. Occasionally I'll use a title (such as "The Outsider"), but the NPC dialogue looks silly.

"What shall we do now, The Outsider?"

Punctuation and capitalization are my friends, so I don't do this TOO often.

The second approach is the "find a name that sounds like you've been gargling with razorblades for a few weeks".

Examples include:

H'rush
Grae-larkhh
Hakk
Khokh

Failing that, there's the ever-popular anagram mode. Find something near your computer, and shift the letters around. I just finished a game with "Fegrita Chetal", which SOUNDS like a good name, until you realize that it's a shuffling of the letters in "Half cigarette".

Well for my first jaunt through the game I was sorely tempted to create a mage named Rincewind (from the Terry Pratchett Discworld series). but I eventually decided that this seemed undignified and ill-fated (for those of you that don't know Rincewind is probably the most inept and cowardly wizard to have ever existed). In the end I created a ranger/cleric whom I named Aragorn (one of the simpler Tolkien names)

ok, there is only one name for the mighty monk that is
YUM-YUM!!!!!!!!!!!

If you're looking for good names, remember, the base language doesn't have to be English. Assuming you know another language, you can use it as the basis for a given race--so that a gnome who would ordinarly be called Grevin Footsnaggle might have some more exotic-sounding name in his (or her, given the number of females in the Footsnaggle line) language.
