CTD's have never happened to me in combat, but yes as i said previously it occurs between transitions, and loading save games.It never happens to me as long as i dont go to the expansion areas like Mournhold or Solsteim. My favorite expansion is Bloodmoon and its so irritating that the game crashes when i run through the landscape as werewolf.
