I mean who (person) do I talk to start the main quest

After you get attacked by the Dark Brotherhood it starts.

Drain and absorb health are temporary effects.

No, they aren't. If you're below your maximum health and acquire more through an absorb health spell, you'll keep all the health you get provided it doesn't go above your maximum. This applies as well to fatigue.

Drain isn't temporary in effect, either. If you're cursed with a Drain Strength attack, for instance, that's permanent unless you get it Restored.

When I got a drain attribute on me, I can get rid of it by just sleeping.
When I got a damage attribute on me, I need to use a spell.

I also peeked through the Construction Set, and drain/absorb were listed as temporary effects.

These come from the main quest that you get from the Temple.
