Random items - what the????

I'm not getting any random items that are mentioned in the walkthrough.
Is my difficulty level to low? I play in the original settings (middle).

Random items are called random items for a reason. You'll only find a small part of the random items available. So don't bother.

i mean a situation when yo should get one of the items that are random but you get none... like when you kill the myrkuls sending you should get a ring and one of the random items and i get only the ring....

The difficulty setting doesn't affect the random loot you get at all. If you only have this problem in one or two situations it's probably just a walkthrough error.

I'm not too sure about that. I played BG:SOA on insane difficulty and a goblin dropped an 8th or 9th level scroll whilst still in Irenicus' crib. Far different from the normal stuff they drop.

This is not the case in IWD2. Extra enemies added in insane or HoF yield sometimes extra loot (which isn't earthshaking, an extra ogre might have an extra winter wolf pelt for example), but nothing else is changed.
