second key

Cannot find the second key to the vault at the fortress. Have looked for hours and killed everything 6 times over. Any hints gratefully received. 
You should have received the key automatically when you killed the plagued lieutenant (the key is a quest item, not something you pick up). So it's possible that you have it but don't realize it.
