Do I need to place 30 points in ranged skill like dodge to use a 30ranged item or do I need to have used my bow and gained 30 levels in it?

At level 50 I'm finding that I need some of these skills less and less and im considering moving to the melee branch for physical resists and hp bonuses.

Equipment requirements are based on class levels and not on skills.

A little bit of multiclassing is a good idea. You only need one level of melee to add points to the fortitude skill.
