Converting 3.0 Books to 3.5

I have the following 3.0 books: Manual of the Planes, Deities & Demigods, Monster Manual II, Psionics Handbook, Epic Level Handbook, Book of Exhalted Deeds, & Book of Vile Darkness. Is there a source that tells relevant rule changes in regard to using them for 3.5 play?

Book of Exalted Deeds is as 3.5 as 3.5 gets. No discussion.

~Manual of the Planes is 3.5, it's now called Planar Handbook.
~Deities and Demigods was updated to 3.5 on the SRD, in Divine Rules.
~Monster Manual II, like all monsters, can easily be tweaked and modified for 3.5 rules. Main thing to remember are things like DR 10/+1 now being DR 10/Magic.
~Psionics handbook was updated to 3.5, and is also available on the SRD. The book is called Expanded Psionics handbook.
~Epic Level Handbook was updated to 3.5 rules on the SRD under Epic rules.
