Anomen holy symbol not appearing bug

This is my first time taking Anomen to level 25 and his holy symbol does not appear properly in his inventory (currently in SoA). He gets a no-name item in his inventory which is unmovable and has no graphic. What is the remedy, how do I get Anomen's holy symbol to appear? I've got fixpack and tweaks both version 9 installed, and Anomen is Sir Anomen having passed his test long ago. Kicking him out of the party and re-joining does not help.
