that morningstar +3/+4 against elves in the severed hand (forgot it's name) has as a special ability: "cold steel". what does that do? if it's like silver in 3e (i.e. better than normal weapons, weaker than +1 weapons) then why did they make it +3?

If we go by old D&D rules it means it can wound elves even if it would normally require a higher enchantmentfactor. So even if an elven baelnorn required +5 weapons a cold steel weapon could still hit it. But I don't actually know.

Only in PnP gaming - AFAIK it has no impact in IWD.

Weird thing about this item, though it says it is Cold Iron, it is not flagged as such in the item flags, and I do not see the effects for Horror so they must be in the exe.

I will take a look and see if I can find a reference to this item in the exe.

(Searched through exe)

Sorry, no luck finding anythng in the exe that references this item. Im sure the info is there, but its all code I am sure, unlike some of the exe that is halfway understandable.
