Proficiencies and Damage

I haven't played D&D games much, more of a diablo fan.
Firstly does anyone have any idead how many proficiencies each class gets by the time they max?
Secondly, and more elaboratly, How on eearth does the damage calculations work, I couldn't find anything on it in the manual. I mena Carsomyr does 1d 12 +5 im guessing that means you roll one 12 sided die, add five, and thats the damage you do. But what on earth does 3d6 +1d6 per level mean, does it mean you roll (3 + char lvl)six sided dies and add the dice values or is it average or does it take the higest?. I cant remember what spell it was but something does 10d6 damgage, again what does this mean???
Thirdly, ive heard a lot of people say carsomyr rocks on the Game banshee strategy section, my question is why?. A monk does 1d20 +3, surely thats more than 1d12+5, and a monk attacks 4 times per round compared to 1.
btw i understand the whole THAC0/ AC deal so that doesnt need explaining
