Suppose you've cast soultrap on something. Now you get your weapon with the same effect (my favorite is 3 secs soultrap + absorb health for the rest of capacity). When you kill the being while the first soultrap is still active you got 2 (two!) souls trapped (assuming you have two proper gems, of course)
