I think the number of attacks depends on how fast you can wield a weapon. Check the weapon's description carefully.

Well..

For melee weapons, the weapon will have a speed factor, typically between 1 and 10 - the lower the number on the weapon, the earlier within the combat round it will strike. This does not, however, affect the number of attacks/round. A few weapons (such as Belm & Impaler) have extra attacks associated with them.

The number of attacks per round is a function of level (1/2 extra attack/x levels - not sure what x is), weapon proficiencies (high weapon skills give you extra attacks), haste (haste gives you +1/2 or +1, improved haste doubles attack rate) and whether or not you're dual wielding. ToB High Level Abilities may give you extra attacks as well.

Missile weapons are similar, but have basic Rates of fire between 1 and 3, unlike all melee weapons which have 1 as their base number.

Sorry I can't be more numerically specific, but that's about all I know.

For levels 1-6 a warrior gets 1 attack/round.
For 7-12 it is 3 attacks/2 rounds
For 13 + it is 2 attacks/round

Specializing in a melee weapon increases this by 1/2 and Grand Mastery increases it by a full attack per round (but I think you need the Baldurdash patch for that to work properly).

Dual wielding adds an additional attack with the off-hand weapon but I don't believe you get any of the other increases.

Thus a 13th level fighter dual wielding with Grand Mastery would get:

1 base attack/round
+1 for 13th level
+1 for Grand Mastery
(for a total of 3 attacks with the main hand)
+1 with the off-hand
for a total of 4 attacks/round.

However, a 10th level fighter dual wielding with Specialization would get:
1 base attack/round
+1/2 for 10th level
+1/2 for Specialization
(for a total of 2 attacks with the main hand)
+1 with the off-hand
for a total of 3 attacks/round.

"Properly" is not the right word here. Grand Mastery was deliberately nerfed not to give another ½ attack.

When I said "properly" I meant according to the D&D rules. I know BioWare nerfed it, but they shouldn't have.

EDIT: Grand mastery works "properly" in BG1 and IWD.
