I've come across a nice item called the belt of inertial barrier which has several effects including a couple of damage resistances (slashing and bludgeoning) but it also has :

"Damage Vulnerability: Piercing 25% Damage Vulnerability"

What does this mean? Do I take an additional 25% damage when hit by a piercing weapon perhaps? If so that seems quite a high price to pay for a couple of "damage resistance 5/-" effects.

Indeed. The extra damage is taken from piercing, but as athe bonus states, the first 5 damage from any other source (bludgeon and slashing) is negated. It's actually a decent little toy.
