NPC quest triggers

Potentially SPOILERISH
Does an NPC's quest trigger from total time in the game or total time an NPC has spent with the party?
Example: Started a new game, kept Jaheira and Yoshimo, and picked up Korgan, Viconia, and Jan. Went to the underdark almost immediately to get Imoen. Now I'm back from the underdark, just finished the interrogation with Elhan and I'm at day 18, hour 12 or somewhere close to there starting chapter 6. I've done the circus tent, slavers quest, korgan's quest, and the de' arnise keep. Sided with Bodhi and did her deeds.
Jan's quest with Lissa hasn't triggered, the Jaheria harper quest hasn't triggered, and not had one romance line out of Jaheira or viccy. I'm also looking to pick up all the other NPC's along the way while doing the remainder of quests in the realms. I'd like to see anomen fail his test, have keldorn burn his wife at the stake, and go through other NPC quests but I don't want to tote these extras around for a long period of time in place of my 'crew' just to trigger their individual quests. So back to the question:
Does an NPC's quest trigger from total time in the game or total time an NPC has spent with the party?

Jan's quest with Lissa hasn't triggered
i remember this taking a long time. Im not certain, but maybe doing trademeet will help.
the Jaheria harper quest hasn't triggered
you have to do the harper quests first, with xzar and monty etc.
not had one romance line out of Jaheira or viccy
romance talks happen every real time hour. Viconia is racist, and only romances elves and half elves.
I think you have to do jaheira's quest to launch into her romance.

as to your question, it is generally time spent with party, except for romances.

I think your answers are at least partially incorrect Mini Me.
1) Viconia only romances humans and half elves (
no pure elves, she's a drow after all, they hate the surface elves

)
2) From my experience NPC quests mostly are dependant on real time after the NPC in question joins the party the first time.
For example you can let Jan join your group, kick him out immediatly and if you take him into your party again after some time his quest will start as soon as you change areas (the messenger appears).
However, some quests (like Jaheira's as you pointed out correctly) require other triggers. The Jaheira romance can only advance if you do the Harper-Quests (poisoned man-->Xzar/Montaron-->Meronia appearing-->etc.).

The way viccy chats her support for the party (rep = 5), you'd think she might have an interest in a dwarven ftr/cl.

The Lissa quest just triggered after leaving elhan's area. Leeloo or whatever his/her name is showed up to talk to jan right after I dispatched of Drizzt and his cronies.
So the answer is both. I might actually have to keep anomen around for a while to trigger his baggage. Keldorn and viccy might need time together to really grow to dislike each other. and so on and so on.
Now I going to go off on a tangent:
What if you're romancing Aerie, with Korgan and Haer'dalis in the party. I suppose again it's a matter of how much time each NPC has spent with the party. It seems to be open to many possibilities though. Anyone know if Haer'dalis will step in to defend Aerie after all the verbal lashings she takes from Korgan.

Viconia is not interested in any kind of dwarf.
You wan't be able to do the harper quest (it trigger very soon in the game and you probably missed to pick up renfeld).
You don't need to keep in the party Anomen and Keldorn, just let them join and then kick them out immediatly, then periodicaly (after a coupple of game-days) let them join again, move between and area to see if their quest trigger (for Keldorn do to gov. district) and kick them out till the quest start.

Game time for some, party time for others. For example you can pick up Jan or Anomen and immediately drop them again from the party. Come back later and pick them up again. There's a good chance their quest will be triggered (if you waited long enough). I do this all the time with NPCs who I don't really want in my permanent party. Nalia is the same way if you've completed the keep quest. Jaheira is a special case. Aerie doesn't really have a quest as you can do the planar prison whether or not you've ever had her in the party. Cernd's quest triggers as soon as you return to Athkatla after completing the druid grove. Mazzy's Copper Coronet dual should trigger as soon as you return to Athkatla from doing the shade lord quest. I don't normally rescue her until just before the final battle with the shade lord and I still get the quest). Her poisoned sister quest seems to be party time based as I never get the quest using the same game time trick that works on Jan and Anomen.

You don't need to actually complete the Sahde Lord quest for Mazzy's duel to trigger.
It seems to have only 2 conditions :
1) party is in front of the copper coronet
2) Mazzy is in the party
