One thing I can tell you about killing Vix'thra right away is that you should wear the Crystal of Undeath, the amulet that you get for freeing the Deva. You'll take a sizeable hit to STR, DEX, and CON, but will gain resistance to Negative Energy, which is what Vix'thra's breath attack hits you with. As with fighting all dragons, you need to have protection from fear and knockdown, too. Vix'thra is pretty much immune to cold, but other elemental damage should work. To give you more specific advice, you should say what class you are playing, though.

I would think that you would be able to take care of him, then. If you've cast all your buffing spells and taken potions to boost your DEX and STR and still can't do enough damage, try to find a weapon that deals bludgeoning damage. Vix'thra is 50% immune to slashing and piercing, but only has normal (+3 Soak 20) damage reduction against bludgeoning. It shouldn't be too hard to find a bludgeoning weapon that carries an enhancement bonus greater than +3 at this point. If you can find an item that grants damage reduction for yourself, that will help too, as Vix'thra's bite and claw attacks deal both piercing and slashing damage.

That one's easy. Either wear something that gives you immunity to mind-effecting spells - like the Cowl of Warding - or pick up a potion of Clarity, which does the same thing. You could also have Deekin cast Clarity on you, if you're letting him tag along. Clarity wears off after a while, though, so check to see how long it lasts.
Actually, I just noticed that Clarity is a 2nd-level bard spell, so you should be able to cast it on yourself.

As a sorceror, I just used lots of Sunbursts.
Is it possible to access his phylactery whilst he's still alive (so to speak
)? Perhaps by the secret passageway?

Yes, that's actually the preferred way unless you want to fight him more than once.
