backstabs are definitely the best way to deal damage. yoshimo at level 9 hit about 160 points of damage (its true) on a giant troll, using his +1 katana.

however, they hit very rarely. is there any way to increase their chance of hitting? magical item maybe?

For backstabbing, thieves get their normal THACO, +4, in order to hit. The +4 is according to AD&D 2nd addition rules, and is based on all attacks that occur from behind receive a +4.
