I have lost character.... he yust wanished. There is no body or anything. What is happening?

It you play at a harder difficulty than normal characters can die permanently if they get too much damage. The character icon just disappears and the items drop on the ground.
Imprisonment even lets the inventory vanish too. There might also be other which work in a similar way.

Disintegration is another possibility. I had a character disintegrated by a Beholder last night funnily enough.
