Everytime you change into the Slayer, it means you're giving in to the evil side of yourself, hence you lose 2 rep each time you change. There are 5 times when you will be injured, set a certain time after you changed into the Slayer, they are:
- after 20 seconds, you lose 5 HP
- 6 seconds later, you lose another 10 - making 15
- 6 seconds later, you lose another 20 - making 35
- 6 seconds later, you lose another 40 - making 75
- 6 seconds later, you lose another 1500

This mean you'll be dead in 44 seconds, if you don't change back to human form.

Hmm...that gave me a new cheese idea, get:
- Protection from Magical Energy
- Belt of Inertial Barrier + Potion of Magic Shielding
- 4/5x Armor of Faith
any can give 100% resistance to magical damage, so the character can survive in slayer mode.
Unfortunately the "slayer weapon" and most of the nice effects are lost (immunities and saving throws). But the slayer form characteristics still remain: including strength and number of attacks. Until the next area change, as it seems area change will end the slayer form.

Really what is the point of the slayer form?
I really never went into that form unless I was forced to. Only one time I can rember that there was a need to get into that form.
So is it really worth the loss of rep points. Was not for me.

Slayer change includes:
+ str: 25, dex: 25
+ ac: -4, base thac0: 0
+ 4 attacks/round, +5 weapon
+ immunity to charm, confusion, sleep, fear, stun, level drain, imprisonment
+ saving throws: 2
+ hit points 100, heal to 100
+ magic resistance: 40%
might a good solution against Kangaxx or Dragons.

I agree. I never used Slayer. this is mostly because I had a good party and wanted to keep my rep, plus there's not too many scrapes that good ol' fashioned hack n'slash couldn't get me out of. I think there were two times when I used Slayer, both because I got in a bind and remembered I hadn't saved for a while. Oh wait, I used it for Kangaxx too. That was great, Slayer ripped the little Morte wanna-be to shreds.

Still though, just knowing you can change into that bad-ass death dealing monster is a cool feeling and good plot device.

I think the Slayer is one of the better balanced things in the game. Certainly not useless, but the costs outweigh the benefits most of the time.

Right on the mark. That balance is critical. Were you able to turn into the Slayer for more extended periods of time, evil groups would be doing it just to relish the stronger attacks and angry militia.

You'll get the ability to change to a Slayer about half way through the game - if you want specific detail, I'll post it. The ability will be in the special abilities section of your character - button on the far right of the bottom panel.

It's no spoiler to say that after Irenicus gets his hands on you for the second time, you while escaping Spellhold, the Slayer ability manifests itself.
