Whilst running through the original campaign for the second time (currently in act 3 at Crossroad Keep) Mephasm seems to ignore the fact that i have already prepared three infernal focuses, the very ones he requested thus putting my dealings with him at a grinding halt. The focuses are equipped in my primary slots yet all Mephasm does is keep inquiring me regarding the "blind sacrifice".
Ideas anyone?

If I understand correctly, do not equip the items. Just have them in your inventory.

Also, if you still have trouble, give your primary character only one at a time when you talk to Mephasm. Having them all at once might confuse him.

IIRC then the dealing with M. is seriously bugged, in more ways then one. So it could just be you ran into a bug with the triggers/conversation etc.
