How is HP calculated?

How is HP calculated? I dont understand. (Lets assume maximum HP at level up).
Lets take my fighter 4/barb 13 for example. He has had 18 constitution for the whole game (+3 modifier). He is not wearing any HP boosting armor. Dalekeeper 2 says my max HP is 208, (and it is higher in game, i know). I want to know how the 208 in Dalekeeper is derived, AND how the in-game HP is derived.
I can't seem to figure this out. The fighter part gives him 4d10 = 40. Barbarian part gives him 13d12 = 156. Total is 40+156 = 196. How is the constitution modifier applied? I dont see why dalekeeper says 208. Where does 208 come from? I assume 208 is WITHOUT con modifier. Correct?
