Unseeing eye quest bug?

I've playing the game for 3rd time, but never crossed a thing like that.
I've retrieved the two half of the quest item, and the unseeing eye is coming for battleing me. But unfortunately he killed my main char, so death animation, and reload game process. Just the fact is that after loading, (and of cuz having the mentioned item), the unseeing eye is not revealing itself for battle. WTF? What should i do??
