After a break from BG i have decided to start a new game. I have finished it once before and i wonder if i should start with some kind of a mod or something that makes it different?

If you have BG2 you could try BG Tutu. It's a pretty massive overhaul of the game; converting it to BG2's engine. There are several quality mods that enhance the NPC relations in BG, as well. BG1 NPC Project is one that comes to mind. Some other mods like BG Unfinished Business, are probably worth your time as well. Check out the modlists at Pocket Plane Planet for others.

I would completely agree with Faust in his recommendations, except to note that EasyTutu has replaced BG1 Tutu in its development. So the latest fixes, etc, are in EasyTutu.

alright, ive installed tutu and i cant see any differents between tutu and the original BG :(

when i installed tut it dropped into an own map "temp". Should i overwrite the original files in BG with the tutu files or does tutu starts auto.. when i start BG?

It is very similar. However things are different. If you run away through a door, your enemies will follow you for example. There are other differences, but the main one as far as I am concerned is that there are lots of mods available to Tutu. Look them up on the sticky section. The other main one is that there are various extra options in charactar building.

You should also notice that hovering your mouse over an "important" building brings up a brief label for it. This won't matter much in the early stages of the game, but once you hit Baldur's Gate itself, you'll find this very useful.

The changes, as the others have indicated, are mainly in terms of AI and character building (you can use all the kits, for instance). But, EasyTutu has a number of mods built for it (the BG2 engine is capable of doing more things than the BG1 engine. Tutu is thus more mod-friendly, as it utilizes the BG2 engine). If you install mods for it like the BG NPC project , you should notice much more fully fleshed out NPCs, as well. The Unfinished Business mod will install some components and quests that were never fully fleshed out in the game. There are a number of other quest mods you can consider, as well. There are a few NPC mods like Finch and Gavin, that will introduce you to modder created characters. Try those things out and you'll have a pretty different game experience.
