Did you kill the four fire elementals by getting them to following you into the ice chamber? If you killed them somewhere else, they respawn. You do not have to kill the fire giant in the ice chamber. I never did.

Did any of you actually finish the game with a solo character? and what class? I always start a new game when I reach underdark.

I can't find Officer Vai. I cleaned out the mine, killed Mulahey, got the reward from the mayor, talked to Tranzig and now I am about to go to Cloakwood forest, but I can't find Officer Vai in Jovial Juggler Inn...
