It's about reflexes and skills. You have to avoid traps and spells. It's not a wild pointed click fest like EQ or Morrowind. In fact you don't even really need to use your mouse a lot unless you need to heal an ally or change view screens. You can auto target the closest enemy by pressing tab, pressing it again targets the second closest and you can quickly flip through the enemies on the field that way. A little faster then using your mouse, which you can use it instead and tab doesn't target allys if you plan to heal one. Moving requires the arrow buttons and your attacks are set on a number bar.
I kill two dwarves in the morning, I kill two dwarves at night. I kill two dwarves in the afternoon, and then I feel alright. I kill two dwarves in time of peace and two in time of war. I kill two dwarves before I kill two dwarves, and then I kill two more.
I may be bad, but I feel good.

But do i select an enemy select attack and then wait untill it is dead?
Or do i select an enemy and have to click 1 for attack every 2 seconds?

You just select it and then move close enough to attack it and wait for you to finish it. That is if you are strong enough to take it on. If you are not strong enough you will constantly be useing special abilieties and running from the monster so that you can take him down.

This is what mostly happens, you attack a monster, it attacks you, you use all the special abeleties you can to get this monster dead as fast as is possible. So it is a click feast but you do not click on the monster, instead you click on your special abileties (adn ofcourse you might occationally want to help out the lon epriest that has to take on a monster all by itself.

There are some attacks that are just click and forget, but those are the most basic ones, like swinging a sword or shooting bows. If you want to use spells or special abilities then obviously you have to click on those special abilities whenever you want to use them.
