Do these items actually do what they say in their description?
I have a ring of smiting that says 3% chance of reducing enemy's health by 96%.
I have had 100's of hits against bosses and have never seen their health reduce by a large amount. Law of averages suggest that it should have happened at least a few times.

I've never played Titan Quest but it's a safe assumption that any proc that powerful won't work on bosses. Have you tried using very weak attacks (unequipping some items maybe) against normal mobs to see if their HP crashes?

The problem is boss has vitality damage resistance. And like Avatar says maybe it won't function against a boss?
Definitely its the first if you are in legendary. You can kill a boss with % red health pretty fast in epic. Legendary I think boss is 96% resistant to % health damage.
