I have killed all the guards at Bann Franderels's estate. There is a door behind a cabinet. How do I get access to this door? Also the exit doesn't work. Do I need to do something behind the cabinet to get the exit to work?

You cannot access that door until later in the game.

You can exit from the same place you entered. If you highlight everything using the TAB key, you'll see an intractable rectangular shape which will allow you to go to the map of Denerim.
