I've got a dual wielding Swashbuckler I'm soloing the game with, but it seems that my off hand weapon is NEVER hitting anyone. No matter what weapon I use, no matter what my THAC0, and no matter how many points I have in that weapon it NEVER hits.
I first noticed this bug when fighting trolls, and using a flame sword in my off hand. I was never able to kill the trolls unless I switched the long sword to my main hand. At first, since I had no skills in long swords I thought it was a THAC0 issue.
Now however, I've gained enough skills to put 2 points in all the weapons I'm using, and 3 points in dual wield style, and I'm STILL not hitting with my off hand weapon.
I even tried putting a magical weapon (The Celestial Fury) in my off hand, and an axe, that I had ZERO skill in once so ever in my main hand, and all of my 50+ hits were done with the axe.
Has anyone ever had this problem before? Or know of any mods that could possibly be causing this issue? I've got a lot of the Ease of Use mods installed, the True Grandmastery mod, and I recently installed the Ascension mod a few months ago, and never noticed this problem on my Kensai.
Edit: I just loaded up my old Kensai in TOB, and have the exact same issue, the off hand weapon never hits. I can't believe I soloed a dual wield Kensai through BG2 without ever noticing this bug.
Edit #2: I uninstalled my weapon styles for all, & True Grandmastery mods, and any other rule change mods related to weapons, and still have the problem. I then uninstalled the ascension mod, but still have the problem. I'm not really sure what other mods I can uninstall to fix this problem, unless I want to try deleting random files in my override folder and seeing if that fixes anything, and I really don't want to do that, it's over 25 MB in size, and I think Bioware's official patches throw files in there to, not just fan mods.
Edit #3: I tried deleting everything in my BG2 override folder (after backing it up of course), but BG2 crashed as soon as I told it to load. And there's 1,325 files in that folder, so yeah, I'm not going to randomly delete some and keep on experimenting, that would take forever.

You should always make two copies of your Override Folder, one before installing any mods. There is a guide to it in the stickied Mods list.
I play a dual-wielding Kensai- Mage- Thief and I have not had the problem. I know this, for I wielded a special "homemade" version of Celestial Fury in my off hand and it worked fine. You can see my numerous mods in the same post, they are too many to list.
Hence, yhough not very helpful, you now know that it is not a general problem.

I'm thinking I might make a backup of my BG2 override, portraits, saves, etc folders and then try uninstalling and reinstalling it, check if it works, install the official patches, and then install the mods one by one till I find the problem.

Well crud, I think my career in the BG2 world is finished.
I tried uninstalling every single ease of use mod I had installed (I even wrote down what ones I had installed). It still didn't fix the problem, so I uninstalled BG2, and then started reinstalling it.
Things got a LOT slower installing it from disc 2, then around 59% the progress bar just about died, an hour later it was at 61%, then I got an error 'Data Cycle (cyclic redundancy check)'.
At first I figured maybe the problem I didn't delete my entire BG2 folder and some leftover files where I installed it, so I deleted the folder, and started an install again. I got the same slowdown at 59%, but got the error much sooner, at 59%.
From what I've read online about the error it looks to me like my old disc #2 is just too scratched up for me to do an install from it anymore.
Edit: Yay, I spoke too soon! I decided to try just a 'recommended' install type, rather than a full install, and it worked. Now if only I can figure out a way to upgrade it to a full install without the installer, I'll have to do some copy and paste experimenting I guess (I have a copy of the entire old & bugged BG2 full install on my desktop).
I created a test character, and had them and Imoen duel each other with a fist in the main hand and a sword in the off hand, while their off hand weapon never hit each other (I'm using weapons on both characters that they have zero skill in, so their THAC0 is like 30 in their off hand), the hit rolls ARE happening for both hands, previously they weren't. attack rolls are displayed in the console. I would first look in the console if there attack actually happens. Also short of .exe modifying mods, of which there are very few, all the rules can be found in the override folder in plain text format. Which is where i'd look second.
off hand weapon only adds maximal 1 attack in 6 seconds (or two if improved hasted) so it could be possible you just didnt notice that during testing, kensai or not.

Sorry to be reviving this after so long, but I just came back to BG2 and fixed this problem, and my unable to reinstall BG2 problem, and wanted to post the answers if someone else was having this problem and found this through a search.
I'd suggest like roller1234 said, turning on the option to always see your hit rolls, to see if you're even getting rolls for both hands in the first place (I wasn't getting rolls at all before when I had this problem, but now I am getting the rolls).
Second, if you have to uninstall and reinstall BG2 for some reason, and have a problem from old discs that aren't readable enough for a full install do the following.
-Make a backup of your entire old BG2 folder and files, even the ones that may be causing you a problem and forcing you to do the reinstall.
-Do a normal or minimum install, whatever your discs will let you.
-Open your newly installed BG2 folder, and copy and paste the 'BG2', 'BG3' and 'BG4' folders into it (and even the BG5 folder if your TOB CD is the problem and won't allow even a normal or min install)
You should now be able to run BG2 just fine as if you did a full install. You may have trouble running the official BG2 patches through, from other registry settings that the full install didn't get to do, but other then that, I haven't had any issues since this fix at getting my BG2 working again. User created mods all should run fine.
