I just resurrected Viconia after defeating Bodhi for the final time--and discovered that she was missing her Amulet of Power. This has occurred in other games in the past, and I have lost other prized possessions from other characters. Has anyone else discovered that things have disappeared? Does anyone have any explanation for the cause?
I reloaded a bunch of saves, and found that the Amulet of Power had disappeared around the time of a visit to Cromwell's Forge. During that visit, I had him make Ankheg plate, Red Dragon armor, and the Crom Faeyr hammer (however it's spelled). I've reloaded saves and had Cromwell make the same equipment again, even cheating in an extra Girdle of Frost Giant and Gauntlets of Ogre Power like I did before [yeah yeah, I know it's cheesy, but the cost is too high for that hammer], but the amulet is still there in my retries.
Of course, I can just cheat in another Amulet of Power, but I'd sure like to know how it disappeared in the first place. Any insights?

Was she wearing it at the time it disappeared? There is a known bug in the game that if your backpack is full the game'll simply dump the item in the top left corner, sometimes it tells you that it's dropped your item on the ground, sometimes it doesn't.

Yes, she was wearing it. Nor was her backpack full at the time. In one save, she's got it on; next save, after Cromwell, it's gone. *poof*
If only level drain disappeared like that!

I never had stuff disappear from my party's inventories like you descried, but a couple of times items were not dropped that should have been dropped (Celestial Fury, for one) and on one occasion, a sword that was used in battle disappeared mid-fight for, as far as I could tell, no reason. (It was the Sword of Roses, or whatever the original name is, the one that adds +3 CHA)

I "lost" my Usuno's Blade +4 the other day. Turns out that since Minsc got petrified in a fight with Beholders, some of the items he was carrying was dumped onto my mainchar, who then got overloaded and dropped the sword on the ground. I didn't notice until awhile later, and had to retrace my step, eventually finding the sword in a pile of dead monsters. Was quite funny.
Usually, if a character "dies" or suffers something like petrification, then he or shield will drop all their containers and dump them on mainchar. If your mainchars inventory is then full, your mainchar will drop items onto the ground, but you usually get the "I can't carry anymore"-message. That's the only thing I can think of, honestly. Yes, I've suffered from that mishap, as well. More than once, I've had Bodhi take Viconia away at the graveyard, dumping her stuff into my main character's overloaded pack--and forgot to pick it up after the battle. I also discovered, the hard way, that the stuff eventually disappears, when I finally got Viconia back and then said, "Now where's all her good loot gone?" Returning to the graveyard days later meant permanently losing it all, or redoing the entire Bodhi-battle and subsequent resurrection in the distant temple.
But this question is not in that arena. Someone above suggested that I'd tinkered with saved files in Shadow Master, or whatever that program is called, but I never do that. It looks like the disappearing loot issue is not common--yet I've encountered it more than just this once. Very strange. Maybe someday I'll take the cover off my computer to clean its innards, and I'll find magic amulets and swords littering the bottom.

If I were you I'd simply cheat back those items with Shadowkeeper - although I understand you may not want to use that program. CLUA works aswell... I mean, losing those items were not intended by the game, I see absoluetly nothing wrong with regaining what was rightfully yours. That's just my 2 gp anyway.
