I have finished Icewind Dale and am ready to move on to Heart of Winter. I already have this installed. How do I get to that part of the game? Once I killed off P. I got the credits. Do I have to start over?

You don't have to replay the whole game. You can start just the expansion from the start screen 
Just pick new game and it should ask what you want. The only problem is you'll have to make new characters, use the premade party or export them from a multiplayer game or dalekeper.

You could also start the expansion from Kuldahar. There is a house near the temple which it is unlock if you have HoW installed. Inside you will find Hjollder, talk to him and you will be able to start your journey into the HoW.
