I've heard of these two items where are they?
The helm of brilliance (i think).
Skull of death or something made from some
madmans parents skulls shoots disentegrate once per day, i think.

Helm of Brilliance - Sold by Ribald in his special collection when you return from the Underdark
This helm is made of polished steel and set with diamonds, rubies and fire opals. The gems glow with an internal light, the source of which can only be magical. The helm makes the wearer resistant to fire as well as giving the wearer some spell abilities. These include fireball, prismatic spray and the sunray.

The mad necromancer-lich N'Ashtar Nikadeemus forged this helmet from the bones of his own parents. His highest ranked general would wear it when he rode into battle, using it to instantly slay the commanders of the opposing army. After that the skeletal armies of N'Ashtar would overrun their demoralized enemies. When N'Ashtar finally fell the Skull of Death was supposedly destroyed.

What happened here was the gradual habituation of the people, ... to receiving decisions deliberated in secret; to believing that the situation was so complicated that the government had to act on information which the people could not understand, or so dangerous that, even if he people could understand it, it could not be released because of national security.

The Skull of Death drops of the mage named Vith(not sure of spelling).

You have to do a stupid quest for him. You retrieve his book of rituals. After you retrieve the book, he asks you to help him kill 3 greater elementals(fire,air,earth)
After the last one, he will give you a rod of abosorption. Ask kindly to recieve more and you will get a few lvl 9 scrolls(I got time stop, wail of banshee, and maybe another or so but i only remember these 2)
Keep demanding more from him and he will fight you. He is very powerful. He casted lvl 9 spells at least 5 times when i fought him and he owned me everytime. So I reloaded and saved at the spot where he would go back to the planes to loot the stuff, and I took my thief and laid snares where he would come back and had Aerie use a sequencer with magic missiles. I used the sequencer as soon as he came back, and he would turn red and get killed by the snares. With the snare/sequencer combo I killed him without taking any damage and he only casted a few spells(stoneskin etc)
