Lost character

I have just been in a fight where 2 in my party died. Actually 3, but the portrait of the last one is completely gone!
Why is that? There isn't even a grey picture. Now i only have 5 characters. What about the last one??
Plz help.

He was reduced to -10 hit points thus unreserectable

Massive damage was delt to him when he died, or "gibbing" him, making him unressurectable.

he could also have been petrified, disintegrated or imprisoned...
