Finding the world in the smallnes of the grain of sand
And holding infinities in the palm of your hand
And Heaven's realms in the seedling of this tiny flower
And eternities in the space of a single hour.

Try and update your graphics driver. Other then that, I don't know.
EDIT: Also, you might want to wait for someone more knowledgable to come along...since I think other people have also solved their graphics problem by reverting to an older driver.
I've never had a problem, so I don't really know how to fix it.

If you could post your system specs, I'd appreciate it (specifically, post your Operating System and Video Card). If you don't know what card you have, you should go to the Control Panel -> System and then open the Device Manager and select Display Adapters.
1) Try rolling back your drivers like Nightmare said (especially if you have a nVidia card) or, in some cases, update to a newer version.

2) If you're using Windows XP, right-click on the Planescape: Torment exe and choose Properties -> Compatibility and use the following settings:
- Run this program in compatibility mode for Windows 98
- Run in 256 Colors (I'm pretty sure PST only has this many?!)
- Run in 640*480 Resolution
- Disable Visual Themes
