Forgot how to change ammo?

If you wish to change ammo from one type to another then I belive the sequence was something like:
Right click the weapon and select the icon that looks like a clip being removed from the gun. (Emptying the gun).
Select new type of ammo and dragging it over the weapon.

New question how to use the different Disk (description says looks like you can use this in your PipBoy) but how?

in inventory, click and hold on it till the open-hand icon and click that
