Victoria's Tomb

Has anyone else gone to Tomb of Victoria and NOT been able to find the mummified head? I cannot enter secret door for the weapons either.

Could someone tell me where Thyaast's phylactery is? I must destroy it so lich doesn't keep coming back.

First--Vics head can be found as you head up the area to the left of the complex you are in and enter the first room to the left...
Second-- the phylactery is in one of the rooms in that area but is behind a secret door---
