Don't you get your ghoul Heather, when you're playing a nosferatu? I just started with one but when I try to talk to her she only says "Please... get a doctor"

The one sure trigger I know for this is to talk to Mercurio about the "vamp blood". You should have had an option for this when you first talked to him, but if you didn't take advantage of it, it should show up again in his conversation topics once you retrieve the Astrolite.
Talking to Knox about ghouls may also do it, though I haven't tried this.

Not a bug
If you don't see Knox out from of the Medical Clinic, it is not a bug. Playing as a Toreador female and Malkavian female I encountered Knox in front of the Medical Clinic. Playing as a Gangrel male and Malkavian male I didn't run in to Knox until I was at The Asylum. There are two possibilities:
* it is random
* dependant on your character's gender
Either way, as someone has already said, you need to speak with Mercurio about the ghouling process (although Knox will volunteer that himself when you speak to him) and you will always find him at The Asylum if you miss him in front of the Medical Clinic.

I've never found Knox in front of the medical center. . .I'll have to look next time. I do know (or think I know) that if you have retrieved the Astrolite, and let Mercurio know about it, then Knox shows up in the Asylum.

As a Gangrel, Malkavian and Nossie males I have found Knox in front of the clinic. I think he leaves once you've retrived the astrolite, because I've spoken to him before entering Mercurio's and on my way to get him some morphine. You don't need the information from him anyways. You can get it from Mercurio just as easily.

Typically when I play, I do not enter the hospital until I've at least talked to Knox first, then Mercurio. You can enter the hospital, the side door, to get the medication for Mercurio before seeing him. Do not do too much else there. Then go see Mercurio, talk to him, give him medicine to help him. Then go back into the front door of the hospital, and do the conversation there. If your computer skills are high enough, bluff your way that way, or other ways.

You'll also need to speak to Dr. Malcolm first (the guy with the dreads in the next room), who'll then tell you he can't do anything for her right now. Only then will you be given the choice to blood her up.

If my memory serves, you talk to Knox, then Dr. Malcolm, then Mercurio, (or vice versa), then finally Heather. You will need the medicine in the hospital to help Mercurio. There might be a lot of going in and out of the side door, for now, of the hospital.

Hm... are you sure these are related? For me it always worked to get infos about ghouling from either Knox or Mercurio and ask Malcom to treat Heather - always worked after that, independent of the info source or the order of inquiries.

I had it out of order, you talk to either Knox or Mercurio first, then Heather, sometimes, then Dr. Malcom. Then you go to Heather to really help her. But the hospital is usually one of my first stops to go into.
