Decapitations only happen when you flourish for (I think) the third consecutive time. The beauty part about decapitating is that, if the head lands in the open, you can kick it around like a soccer ball, or as it were, a chicken.
Get up, stand up. Come on, throw your hands up.

you're right there.
*to the other guys*
I suppose I brought that on myself by failing to mention that I was only refering to decapitations with melee weapons. I will not, however apologize for I don't think that you have done anything substantial enough to gain the merit.
Use a thesaurus if you don't (and I'm almost certain some of you wouldn't) know what I'm talking about.

I've decapitated people with enflame before. They were stunned on their feet and I cast it, and their bodies dropped but their heads flew upwards a couple feet before dropping. It was pretty cool.
And it seemed a little unclear before, but to decapitate with a melee weapon they have to be stunned on their feet then you hit them with a flourish.

also if you are in berserk you do not need to flourish and they do not even need to be stunned!

decapitation...
an enemy has to be stunned before you can decapitate them... or so i've found! =P
with a Melee Weapon, use "Flourish"... no matter what melee weapon you have equipped, the head will come off! =D
with a bow, click the right thumbstick twice, to get a nice view of the enemy... best way i've found to get a "more-than-certain" headshot, is to aim just above the head, making sure the crosshair is still red... then let it rip! =) obviously, the higher your Accuracy Skill, the easier it is to aim!
when using "Lightning", just keep your thumb on whatever button you've assigned "Lightning" to, if you watch carefully, for a split-second the enemy/enemies will be stunned and "pop", off come their heads... as with aiming, better if your "Lightning" Skill is of high level!
hope that helps, and maybe clears up some confusion! =)
