Umm... maybe try a noclip. Open the console (to get access to that, you're going to have to edit the shortcut in your start menu to point to the whatever it says, but add "-console" without quotes. After that, press the ~ to get access ingame) and type "noclip," no quote. Run through the door, type it again to disable, and there you go.

install the patch...you may even try restarting the game...just be sure that if you do reload, make it beofre the whole Jack thing or it won't do squat(though you are early enough in the game where you can just restart without too much difficulty)...Hope this helps(and the noclip thing didn't work? do the whole console thing mentioned in other treads and try that if nothing else...)

add the -console to the start command in the shortcut on your desktop.
should read like this:
"C:\Program Files\Activision\Vampire - Bloodlines\vampire.exe" -heapsize 512000 -console
(if you installed to the same directory, just copy and paste)
when done successfully you should see a console box once you start the game again.
click it away and start playing.
now the important stuff:
when you want to enter someting in the console get to menu (hitting ESC). if you bring up the console ingame it will not respond anymore.
once you have entered your desired commands simply hit ESC again and you are back on track
Hope this helps!

Well, here's the last idea I have. Keep in mind you can't stand on the grate as you open it. You have to stand beside it, about 2 feet away, and get as far as possible with the icon still appearing. Then, and only then, will the hatch open. Sounds like this is your problem, if reloading isn't fixing it.
