I just finished killing the plaguebearer.
Im leaving the sewers.... and I crawl up the manhole.
I Hit The "action" button to leave.
it loads Downtown, but I fall down below the city instead of spawning on land!!!!
Help!

The exact same thing happened with me i kept trying everything but i came to a conclusion You can't do anything about it 
This really annoyed me and the only thing i can tell you to do is to start the game all over again every other time i got to this point it was fine but sorry other than that you might be able to use the Noclipp cheat so you could get back up to the city i don't know if this will work though. good luck
